Can Young Women Keep Their Fertility After Early Ovarian Cancer?

For young women with early-stage ovarian cancer, preserving fertility is often a top concern. Fertility-sparing surgery (FSS) allows doctors to remove the cancer while keeping one ovary and the uterus. But does this affect survival?

This study looked at nearly 1,800 women with early-stage epithelial ovarian cancer (EOC). All were under 50 and had tumors limited to one side or in early stages (stage 1 or 2). About 1 in 5 had fertility-sparing surgery. The rest had standard surgery, which removes both ovaries and the uterus.

After carefully matching patients with similar cancer details, researchers compared survival between the two groups. They found no difference in cancer-specific survival at 3, 5, or even 10 years.

Even when looking at subgroups—based on age, race, tumor type, or whether chemotherapy was used—FSS did not lead to worse outcomes, except for a small group with more advanced stage 2 cancer.

This means that for many young women with early-stage ovarian cancer, keeping the chance to have children later is possible—without increasing the risk of the cancer coming back or affecting survival.

Doctors may now consider expanding the use of fertility-sparing surgery for women who want to preserve fertility.
