Fertility-Sparing Surgery Can Be Safe for Women With Advanced Ovarian Tumors

Many young women with borderline ovarian tumors (BOTs) want to keep their ability to have children. But for those with advanced disease, is fertility-sparing surgery (FSS) still a safe choice?

This study looked at 86 women treated over several decades for advanced BOTs. Most had tumors in both ovaries and more than half were in stage III. Instead of removing both ovaries and the uterus, doctors used a fertility-sparing approach in all cases.

The results were promising. Even though about 77% of women had the cancer come back at least once, it usually came back in the ovaries and could be treated again. In fact, many women were able to have a second or even third surgery while still keeping their fertility.

Only three women developed a more serious form of cancer later on. At the end of the study, nearly all women (96.5%) were alive and free of disease.

Certain factors, like having tumors on both ovaries or the type of surgery used, slightly increased the chance of recurrence. But overall, the treatment was safe.

This study supports giving young women with advanced BOTs the option to preserve fertility. Even with a higher chance of recurrence, long-term outcomes remain excellent.
