Small lumps found in the testicle are called small testicular masses. In the past, doctors often removed the entire testicle to treat these masses because of the fear of cancer. New international guidelines suggest that this may not always be necessary.
Researchers reviewed the latest recommendations from major medical organizations, including European and North American cancer and urology societies. These guidelines examined the best way to manage small testicular masses while protecting both health and quality of life.
The review shows that testis-sparing surgery is becoming an important treatment option for carefully selected patients. In this approach, surgeons remove only the small mass and keep the rest of the testicle intact.
This method may be considered when imaging tests show a small or unclear mass and tumor marker blood tests are negative. It can be especially helpful for men who have only one testicle or tumors in both testicles, because preserving testicular tissue helps maintain hormone production and fertility.During surgery, doctors can perform a rapid tissue test called a frozen section analysis. This allows them to check whether the mass is cancerous while the operation is still in progress. If the mass turns out to be benign, the surgeon can avoid removing the entire testicle.The new guidelines show growing agreement among experts that testis-sparing surgery can safely treat selected small testicular masses while preserving normal testicular function.These recommendations encourage doctors and patients to discuss all treatment options carefully and choose the approach that best balances cancer control with long-term health and fertility.
