Exploring PIPAC for managing platinum resistant and refractory ovarian cancer with peritoneal spread A collaborative multi-institutional study.DOI 10.1016j.ejso.2025.111161

Some women with ovarian cancer stop responding to common treatments like platinum-based chemotherapy. When this happens, the cancer can grow quickly and spread inside the belly, causing pain, swelling, and other problems. A new study looked at a treatment called PIPAC to see if it could help.

PIPAC stands for Pressurized IntraPeritoneal Aerosol Chemotherapy. It delivers cancer medicine as a fine mist directly into the belly during a short surgery. This way, the medicine reaches the cancer more directly, using lower doses with fewer side effects.

In this study, 40 women with hard-to-treat ovarian cancer joined from 2016 to 2023. Most of them were able to finish their planned PIPAC treatments. The treatment was safe, and no severe side effects were seen. After treatment, over half of the women in one group had some benefit—either their cancer stayed the same or got smaller.

Many women also had relief from swelling in the belly (ascites) after just one PIPAC cycle. Three women who couldn’t have surgery before became able to have it after PIPAC. Some women lived over a year after starting treatment, and one group had a survival of up to 17 months.

PIPAC won’t cure this type of cancer, but it may help manage symptoms, slow down the disease, and improve quality of life. Doctors say it could be a useful option when other treatments stop working.
