A swollen scrotum is often linked to harmless conditions. In rare cases, it can be a sign of cancer. This case report describes a man who noticed scrotal swelling that later turned out to be a very uncommon cancer called liposarcoma.

Liposarcoma is a cancer that grows from fat tissue. It is rare. Only a small number of people are diagnosed each year in Nordic countries. What made this case unusual was how far the tumor had grown before it was discovered.

The tumor started in the scrotum. Over time, it grew upward through the inguinal canal, which is a natural passage between the abdomen and the groin. From there, it extended deep into the abdomen, an area known as the retroperitoneum. It also grew close to the left kidney. This type of growth can happen slowly and may not cause strong pain early on.

Doctors treated the patient with radical surgery. This means the goal was to remove the entire tumor in one operation. Surgeons carefully removed the mass from the scrotum, abdomen, and the area near the kidney. After surgery, tissue testing showed a dedifferentiated liposarcoma. This is a more aggressive form of the disease. The good news was that the tumor was removed with clean margins, meaning no cancer cells were seen at the edges of the removed tissue.

This case shows how large and extensive liposarcomas can become if not detected early. It also shows that surgery can still be effective, even when the tumor spreads across multiple areas.

For patients, this report highlights an important message. Any scrotal swelling that grows, feels firm, or does not go away should be checked by a doctor. Early assessment can lead to earlier treatment and better outcomes, even for rare cancers like liposarcoma.
