The retroperitoneum includes the kidneys, adrenal glands, ureters, major blood vessels such as the aorta and inferior vena cava, and parts of the pancreas and duodenum.

Because the retroperitoneum is relatively spacious and lies deep within the abdomen, tumours arising in this area may grow silently for a long time. Some patients may not experience obvious symptoms until the tumour becomes large or starts pressing on nearby organs, nerves, or blood vessels.

Retroperitoneal tumours are uncommon and can include different types of sarcomas, nerve-related tumours, lymphomas, and other tumours. Their diagnosis and treatment can be complex because of their close relationship with major blood vessels and vital organs.

In this video, we explain the main types of tumours that can occur in the retroperitoneum. Tumours may develop from the fat within this space, ranging from benign tumours such as lipomas to malignant tumours such as liposarcomas. They can also arise from nerves and their supporting tissues, resulting in nerve sheath tumours.

Muscles and connective tissues can give rise to different types of soft-tissue sarcomas. Lymphatic tissues may also be involved, particularly in conditions such as lymphoma.

Importantly, some tumours found in the retroperitoneum actually originate from organs located within or adjacent to this space. For example, the kidneys, adrenal glands, and pancreas can develop their own specific types of tumours.

As the tumour increases in size, patients may develop abdominal pain, back pain, abdominal fullness, or visible swelling. Because the tumour can press against nearby organs, nerves, blood vessels, or other structures, additional symptoms may develop. These can include urinary problems, changes in bowel habits or other digestive symptoms, and swelling of the legs if major blood vessels or lymphatic pathways are compressed.

Not every retroperitoneal tumour causes symptoms. In some patients, the tumour is discovered incidentally during an ultrasound, CT scan, or MRI performed for an unrelated medical problem.

The important message is that persistent, unexplained, or progressive abdominal or back symptoms should not be ignored. While these symptoms are commonly caused by many less serious conditions, persistent symptoms may require appropriate clinical evaluation and imaging.

The first step is usually imaging. A CT scan or MRI helps doctors understand the tumour’s size, exact location, extent, and relationship with nearby organs, nerves, and major blood vessels. This information is particularly important when planning further investigations and treatment.

In many patients, a biopsy may be recommended. A small sample of tissue is obtained and examined by a pathologist to determine the exact type of tumour. Knowing whether the tumour is a sarcoma, lymphoma, germ cell tumour, or another type is essential because treatment differs significantly between these conditions.

Blood tests may also be useful in selected patients. Tumour markers such as AFP, beta-hCG, and LDH may be checked when a germ cell tumour is suspected.

Robotic surgery provides surgeons with enhanced visualization, precise instrument control, and the ability to work through small incisions. For appropriately selected patients, this minimally invasive approach may offer benefits such as smaller incisions, less postoperative pain, reduced blood loss, and faster recovery.

However, not every retroperitoneal tumour is suitable for robotic surgery. The size and exact location of the tumour, its relationship with the kidneys, bowel, pancreas, ureters, aorta, inferior vena cava, and other major blood vessels, as well as whether these structures are involved, all influence the surgical approach.

For some large, complex, or locally advanced tumours, open surgery may provide better exposure and allow safer and more complete tumour removal.

The retroperitoneum contains important structures such as the aorta and inferior vena cava, which are major blood vessels supplying and draining the body. It is also closely related to organs including the kidneys, pancreas, and duodenum. The ureters, which carry urine from the kidneys to the bladder, and important nerves may also lie very close to the tumour.

As a retroperitoneal tumour grows, it may displace, compress, or sometimes involve these surrounding structures. This makes surgical planning extremely important. Detailed imaging with CT or MRI helps the surgical team understand the tumour’s exact location and its relationship with surrounding organs and blood vessels.

The goal of surgery is to achieve complete tumour removal with appropriate oncological margins, while preserving the function of critical organs and structures whenever it is safe and feasible.

In carefully selected patients, complete tumour removal may still be possible. When the tumour involves an adjacent organ or structure, surgeons may perform an en-bloc resection, removing the tumour together with the involved structure as one specimen. The goal is to achieve the best possible oncological clearance while maintaining patient safety.

Depending on what needs to be removed, reconstruction may also be required. This can include reconstruction of major blood vessels, repair or reimplantation of the ureter, or reconstruction of the digestive tract.

Because these operations can be highly complex, treatment planning often involves a multidisciplinary team, including surgical oncologists, vascular surgeons, urologists, colorectal surgeons, radiologists, pathologists, medical oncologists, and other specialists as required.

मांसपेशियों (Muscles) और संयोजी ऊतकों (Connective Tissues) से विभिन्न प्रकार के सॉफ्ट-टिश्यू सार्कोमा (Soft-Tissue Sarcomas) हो सकते हैं। लिम्फेटिक टिश्यू (Lymphatic Tissues) भी प्रभावित हो सकते हैं, विशेष रूप से लिम्फोमा (Lymphoma) जैसी बीमारियों में।

महत्वपूर्ण बात यह है कि रेट्रोपेरिटोनियम में पाए जाने वाले कुछ ट्यूमर वास्तव में इस क्षेत्र में मौजूद या इसके पास स्थित अंगों से उत्पन्न होते हैं। उदाहरण के लिए, किडनी (Kidney), एड्रिनल ग्लैंड (Adrenal Gland) और पैंक्रियास (Pancreas) में भी अलग-अलग प्रकार के ट्यूमर विकसित हो सकते हैं।

जैसे-जैसे ट्यूमर का आकार बढ़ता है, मरीज को पेट में दर्द, कमर में दर्द, पेट में भारीपन या पेट में सूजन महसूस हो सकती है। ट्यूमर आसपास के अंगों, नसों, रक्त वाहिकाओं या अन्य संरचनाओं पर दबाव डाल सकता है, जिससे कुछ और लक्षण भी हो सकते हैं। इनमें पेशाब से जुड़ी समस्याएँ, मल त्याग में बदलाव या अन्य पाचन संबंधी समस्याएँ और पैरों में सूजन शामिल हो सकती है। यदि ट्यूमर बड़ी रक्त वाहिकाओं या लसीका मार्गों पर दबाव डालता है, तो पैरों में सूजन हो सकती है।

हर रेट्रोपेरिटोनियल ट्यूमर में लक्षण दिखाई दें, ऐसा जरूरी नहीं है। कुछ मरीजों में यह ट्यूमर किसी अन्य स्वास्थ्य समस्या के कारण किए गए अल्ट्रासाउंड, CT स्कैन या MRI के दौरान संयोग से पता चलता है।

महत्वपूर्ण बात यह है कि पेट या कमर में लगातार, बिना किसी स्पष्ट कारण के या बढ़ते हुए लक्षणों को नजरअंदाज नहीं करना चाहिए। हालांकि, ऐसे लक्षण कई कम गंभीर बीमारियों के कारण भी हो सकते हैं, लेकिन यदि ये लगातार बने रहें, तो डॉक्टर से उचित जांच और आवश्यक इमेजिंग करवाना जरूरी हो सकता है।

सबसे पहले आमतौर पर इमेजिंग (Imaging) की जाती है। CT स्कैन या MRI से डॉक्टर ट्यूमर का आकार, सटीक स्थान, फैलाव और आसपास के अंगों, नसों तथा प्रमुख रक्त वाहिकाओं से उसके संबंध को समझने में मदद मिलती है। आगे की जाँच और उपचार की योजना बनाने में ये जानकारी बहुत महत्वपूर्ण होती है।

कई मरीजों में बायोप्सी (Biopsy) की आवश्यकता हो सकती है। इसमें ट्यूमर से ऊतक (Tissue) का एक छोटा सा नमूना लिया जाता है और पैथोलॉजिस्ट (Pathologist) उसकी जाँच करके ट्यूमर के सही प्रकार की पहचान करते हैं। यह जानना महत्वपूर्ण है कि ट्यूमर सार्कोमा (Sarcoma), लिम्फोमा (Lymphoma), जर्म सेल ट्यूमर (Germ Cell Tumour) या किसी अन्य प्रकार का है, क्योंकि इन सभी का उपचार अलग-अलग होता है।

कुछ मरीजों में ब्लड टेस्ट भी आवश्यक हो सकते हैं। यदि जर्म सेल ट्यूमर का संदेह हो, तो AFP, beta-hCG और LDH जैसे ट्यूमर मार्कर (Tumour Markers) की जाँच की जा सकती है।

रोबोटिक सर्जरी में सर्जन को बेहतर तरीके से देखने, सर्जिकल उपकरणों को अधिक सटीकता से नियंत्रित करने और छोटे चीरे (incisions) के माध्यम से ऑपरेशन करने की सुविधा मिलती है। सही तरीके से चुने गए मरीजों में इस मिनिमली इनवेसिव (minimally invasive) तकनीक के कई फायदे हो सकते हैं, जैसे छोटे चीरे, ऑपरेशन के बाद कम दर्द, कम रक्तस्राव और जल्दी रिकवरी।

हालांकि, हर रेट्रोपेरिटोनियल ट्यूमर (Retroperitoneal tumour) का इलाज रोबोटिक सर्जरी से नहीं किया जा सकता। ट्यूमर का आकार और सही स्थान, तथा उसका किडनी, आंत, अग्न्याशय (pancreas), मूत्रवाहिनी (ureter), महाधमनी (aorta), इन्फीरियर वेना कावा (inferior vena cava) और अन्य बड़ी रक्त वाहिकाओं के साथ संबंध—साथ ही इन संरचनाओं में ट्यूमर के फैलाव—सर्जरी की तकनीक तय करने में महत्वपूर्ण भूमिका निभाते हैं।

कुछ बड़े, जटिल या आसपास के अंगों में फैले हुए ट्यूमर में ओपन सर्जरी बेहतर तरीके से सर्जिकल क्षेत्र को देखने और सुरक्षित एवं पूरी तरह से ट्यूमर निकालने में अधिक मदद कर सकती है

जैसे-जैसे रेट्रोपेरिटोनियल ट्यूमर बढ़ता है, यह आसपास की इन संरचनाओं को हटा सकता है, दबा सकता है या कभी-कभी उनमें फैल भी सकता है। इसलिए सर्जरी की योजना बहुत सावधानी से बनाना आवश्यक होता है। CT या MRI जैसी विस्तृत इमेजिंग से सर्जिकल टीम को ट्यूमर की सही स्थिति और आसपास के अंगों तथा प्रमुख रक्त वाहिकाओं के साथ उसके संबंध को समझने में मदद मिलती है।

सर्जरी का उद्देश्य उचित ऑन्कोलॉजिकल मार्जिन (Oncological margins) के साथ ट्यूमर को पूरी तरह निकालना होता है, साथ ही जब यह सुरक्षित और कैंसर के उपचार के दृष्टिकोण से उचित हो, तब महत्वपूर्ण अंगों और संरचनाओं के कार्य को संरक्षित रखना भी उतना ही महत्वपूर्ण है।

चुनिंदा मरीजों में ट्यूमर को पूरी तरह निकालना संभव हो सकता है। जब ट्यूमर किसी आसपास के अंग या महत्वपूर्ण संरचना से जुड़ा होता है, तो सर्जन एन-ब्लॉक रिसेक्शन (en-bloc resection) कर सकते हैं। इसमें ट्यूमर और उससे जुड़े प्रभावित अंग या संरचना को एक साथ निकाला जाता है। इसका उद्देश्य ट्यूमर को पूरी तरह हटाकर बेहतर कैंसर नियंत्रण (oncological clearance) प्राप्त करना और मरीज की सुरक्षा बनाए रखना होता है।

ट्यूमर के कारण किन संरचनाओं को निकालना पड़ा है, इसके आधार पर रिकंस्ट्रक्शन (reconstruction) की आवश्यकता भी हो सकती है। इसमें बड़ी रक्त वाहिकाओं का पुनर्निर्माण, यूरेटर (मूत्रवाहिनी) की मरम्मत या उसे दोबारा जोड़ना, अथवा पाचन तंत्र की पुनर्रचना शामिल हो सकती है।

क्योंकि इस प्रकार की सर्जरी बहुत जटिल हो सकती है, इसलिए उपचार की योजना अक्सर मल्टीडिसिप्लिनरी टीम (multidisciplinary team) द्वारा बनाई जाती है। इसमें सर्जिकल ऑन्कोलॉजिस्ट, वैस्कुलर सर्जन, यूरोलॉजिस्ट, कोलोरेक्टल सर्जन, रेडियोलॉजिस्ट, पैथोलॉजिस्ट, मेडिकल ऑन्कोलॉजिस्ट और आवश्यकता के अनुसार अन्य विशेषज्ञ शामिल हो सकते हैं।
