IN SHORT
Stage I means the cancer stayed inside the testis, and the markers settled after it was removed. Most men then need no further treatment at all.
There are two sound paths. One is close watching with markers and scans. The other is one short course of treatment, given once, to lower the chance of return.
The tumour type, one or two features in the report, and whether you can keep every visit decide which path suits you. The tumour board helps you choose.
This page begins after the testis has been removed through the groin. The markers taken after the operation have come down to normal. The CT scan shows no spread. That is stage I. How the stage is worked out is explained in testicular cancer stages and risk groups. This page covers only the next decision: what, if anything, comes after the operation. Testicular cancer usually responds well to treatment, even after it has spread. That is why the aim at stage I is to avoid giving treatment a man does not need, without letting anything slip.
Close watching avoids treating men who are already free of cancer. One short course lowers the chance of return, but every man who takes it gets the drug.
| Question | Close watching (surveillance) | One short course |
|---|---|---|
| What happens now? | No more treatment. Markers, check-ups and scans at set times. | One dose of carboplatin for seminoma, or one cycle of BEP for non-seminoma. |
| What does it ask of you? | Every visit, every scan, for years. Missing them is the main danger. | The drug is given once. Follow-up visits still continue. |
| What is the gain? | Men already free of cancer are spared drug side effects. | A lower chance that the cancer comes back. |
| If the cancer comes back? | It is usually found early and treated then. | Less likely, but still possible, so checks continue. |
For stage I seminoma, close watching is the recommended path. One dose of carboplatin is a less preferred choice, kept for a few clear reasons.
For stage I non-seminoma, close watching is again preferred for most men. The feature that changes the advice is spread into small vessels in the testis.
The pathologist looks for cancer cells inside small blood or lymph vessels of the testis. This is called lymphovascular invasion, or LVI. When LVI is present, the chance of return is much higher. One cycle of BEP chemotherapy then lowers it a great deal, and is discussed as a real option. When LVI is absent, surveillance is usually the better fit.
An operation to remove the nodes at the back of the abdomen (RPLND) is used only in selected men at this stage. One example is a teratoma that contains another kind of cancer. Another is a man who cannot follow surveillance and does not want chemotherapy. Where it is done, nerve-sparing technique is used to protect ejaculation. More is on the testicular cancer surgery and fertility page.
Surveillance is a treatment, not a pause. It works only when markers and scans happen on time, so your real-life chances of attending are discussed openly.
| Question | Why it matters |
|---|---|
| How far do you live from the clinic? | Visits every three months in the first two years mean many trips. |
| Will work take you away from Gujarat? | A move can break the schedule without anyone noticing. |
| Can the clinic reach you by phone? | A missed visit must be followed up quickly. |
| Is there someone at home who can remind you? | Years of check-ups are easier with support. |
If the honest answer is that visits will be hard, the single dose is a sensible choice, not a lesser one. The full schedule is in testicular cancer follow-up.
Some steps come first, whichever path you take:
A return found on surveillance is usually small and found early. It still usually responds well to treatment, even after it has spread.
A marker that rises is never simply watched. You are rescanned, and the tumour board looks at your case within the same week. Treatment then follows the plan for spread disease, explained in treatment of spread testicular cancer. This is the reason surveillance is safe only when it is kept.
Most men do not. Close watching is the preferred path for most stage I patients. One short course is discussed when the report shows certain features, or when regular visits are unlikely.
It is safe when every visit is kept. Markers and scans find a return early, and it usually responds well to treatment, even after it has spread.
It is no longer a routine first choice. It can cause new, different cancers in the treated area years later, and surveillance or one dose of carboplatin work without that risk.
LVI means cancer cells were seen inside small vessels in the testis. In non-seminoma it raises the chance of return, so one cycle of BEP is discussed.
Yes. Sperm banking is offered to every man before chemotherapy or node surgery, and the answer is written down. Even one sample is worth storing.
