IN SHORT
Often not. A kidney tumour is planned on a CT taken in several phases, including one before the dye, plus a chest CT.
Kidney function is measured properly before any decision. One creatinine value is not enough, because the operation choice depends on the kidney you keep.
A biopsy is not routine. It is done when its answer would change the plan, such as before ablation or before medicines.
A single CT, taken only after the dye, can make a harmless lump look like cancer. A cyst that has bled, or a fatty growth called an angiomyolipoma, can be read as a tumour. So the kidney CT is taken in several phases. One set of pictures is taken before the dye. Others follow as the dye reaches the arteries, fills the kidney and then drains out. Comparing them shows whether the lump truly picks up dye, which is what a solid tumour does. Planning surgery on a one-phase CT is one of two errors the practice’s protocol names at this step.
A good report is more than the tumour’s size. It maps the kidney, its blood vessels and the other kidney, because each detail changes the operation.
| Detail | Why it matters |
|---|---|
| Size of the tumour. | It sets the stage and the choice between partial and whole kidney removal. |
| A complexity score and how deep it sits. | A tumour bulging outward is simpler to remove than one buried deep. |
| Contact with the centre of the kidney. | Closeness to the main vessels and the urine-collecting area makes partial removal harder. |
| Number of kidney arteries. | The surgeon needs this map before clamping. |
| A tumour clot in the kidney vein or main body vein. | How far up it reaches decides who else joins the operation. |
| Lymph nodes and the adrenal gland. | These show whether the disease has gone beyond the kidney. |
| The other kidney. | Its health decides how much kidney must be saved. |
The second error the protocol names is missing a clot in the main vein before the abdomen is opened. So the report must say whether a clot is there and how far up it goes.
The key fact noted at diagnosis is often not the stage. It is how well the other kidney works, and how much reserve will remain.
A single creatinine report is not a kidney assessment. That is why this practice uses split-function scans more often than many guidelines ask. The numbers make the talk about kidney reserve honest. If kidney function is already reduced, a kidney specialist is involved and saving part of the kidney is strongly preferred. The choice itself is explained on the partial or whole kidney removal page.
No. A clearly cancerous kidney mass in a fit patient usually goes to surgery without one. A biopsy is done when its result changes the next step.
| Situation | Biopsy? |
|---|---|
| Before heat or freezing treatment (ablation). | Yes. It is done as a separate, earlier procedure. |
| Before medicines for cancer that has spread. | Yes. The type guides the medicine. |
| An older patient with other illnesses, before surgery is chosen. | Often, when the answer would change the plan. |
| Lymphoma or spread from another cancer seems possible. | Yes. |
| A small mass that is going to be watched. | Sometimes, when the type would change the watching rules. |
| A clearly cancerous mass in a fit patient. | Not needed before the operation. |
Cysts are graded on CT using the Bosniak system. Class I and II cysts need no follow-up. Class IIF cysts are watched with scans. Class III and IV cysts are treated as cancer. If you want to know how a kidney tumour first shows itself, read how kidney cancer shows itself.
Some findings call for preparation before major surgery. These include a large recent weight loss, very low body weight, low blood albumin or weak kidney function. Any one of them leads to a fitness programme and an early anaesthetic review. Walking, breathing exercises, protein and stopping smoking start from the first visit. A genetic referral is made when the patient is under 46, when both kidneys or several spots are affected, or when a close relative had kidney cancer.
A CT taken only after the dye cannot show whether a lump truly picks up dye. A multiphase CT can, and it maps the vessels too.
No. Ultrasound often finds the lump first. Surgery is planned on a multiphase contrast CT of the abdomen and a CT of the chest.
Usually not, if the mass is clearly cancerous and you are fit. A biopsy is done before ablation, before medicines, or when it changes the plan.
It shows how much work each kidney does. That matters when deciding how much kidney can safely be removed.
Only if there are symptoms, such as new bone pain or headaches, or if the tumour looks high risk on the first scans.
The first signs of kidney cancer are on how kidney cancer shows itself. The operation choice is on the partial or whole kidney removal page. For questions, call +91-63590-11009.
