For men with advanced prostate cancer that no longer responds to hormone therapy (metastatic castration-resistant prostate cancer, or mCRPC), treatment options are limited. Standard medicines like enzalutamide can slow the disease, but many patients still see their cancer progress. A large international study has now shown that combining enzalutamide with another drug, talazoparib, helps men live longer.

The TALAPRO-2 trial included more than 800 men from 26 countries. Half received enzalutamide with talazoparib, while the other half received enzalutamide with a placebo. Patients were followed for more than four years.

The results were clear. Men who received the combination lived an average of 46 months, compared to 37 months for those on enzalutamide alone. The benefit was strongest in men whose tumors had specific DNA repair mutations (HRR-deficient cancers), where survival improved significantly. Even in men without these mutations, the combination helped delay cancer growth for much longer—about 33 months compared to 20 months.

The main side effect was anemia (low red blood cell count), which affected nearly half of the men in the talazoparib group. Some also developed low white blood cell counts. These side effects are important to monitor but were considered manageable with medical care.

This study shows that talazoparib plus enzalutamide is more effective than enzalutamide alone and should be considered a new standard treatment for men with mCRPC, especially those with HRR gene mutations.

For patients, this offers new hope—not just for longer survival, but also for better control of advanced prostate cancer.
