IN SHORT
Most testicular cancers follow one path: blood markers, removal of the testis through the groin, then a staging scan. A few situations change it.
A tumour in both testes, or in the only testis, may allow removal of the tumour alone. Frozen section and a tumour board plan come first.
Very high hCG with breathlessness is an emergency. Chemotherapy starts at once, before the testis is removed, because bleeding in the lungs can threaten life.
For most men the order is clear. Blood markers are taken first. The testis is removed through a cut in the groin. A CT scan then shows the stage. The order is set out in testicular cancer tests in order.
Some men do not fit that pattern. Their case needs a different order, a different operation, or the tumour board before anything else. This table shows the main ones:
| Situation | What changes |
|---|---|
| Sudden pain with a swollen testis. | A twist must be ruled out within the hour. Pain does not rule out a tumour. |
| Very high hCG with breathlessness. | Chemotherapy comes first, urgently. The testis is removed later. |
| Tumour in both testes, or in the only testis. | Removing the tumour alone may be possible, in a planned operation. |
| The other testis is small or was undescended. | A biopsy of the other testis is discussed. |
| An earlier operation or biopsy through the scrotum. | Staging and follow-up change. |
| Cancer outside the testis, or a different tumour type. | A separate plan, decided by the tumour board. |
| Regular check-ups will be hard to keep. | One dose of chemotherapy may be discussed instead. |
A sudden, painful testis in a young man may be a twist of the cord (torsion). That cannot wait for a scan tomorrow. The decision to explore it is made within the hour. Pain does not mean it is not cancer. Bleeding inside a tumour can also be painful. So a painful lump still gets the full work-up. A swelling labelled as fluid or infection that has not settled after one course of antibiotics should be scanned, not treated with a second course.
When hCG runs into the thousands with breathlessness or a large mass in the abdomen, chemotherapy starts urgently. The operation on the testis waits, because the lungs come first.
This is a medical emergency. Fast-growing tumour in the lungs can bleed. Waiting for the operation would lose time the man does not have. A few other problems also need the surgeon the same day:
Same-day emergencies in testicular cancer:
Removing the whole testis is the standard operation. In a few men it would leave no testis at all. Then removing only the tumour (partial orchidectomy) may be considered. This is done only in a man with tumours in both testes, a single testis, or a small tumour in a working single testis. The tissue is checked during the operation (frozen section), and the plan is agreed by the tumour board first. Sperm banking is offered before any of this. How the operation is done is covered in testicular cancer surgery: groin, not scrotum.
A testis that did not come down as a child, an earlier tumour in the other testis, a shrunken testis and infertility all raise the risk. The scan always checks both testes.
When the other testis is small or was undescended, or the man is young, a biopsy of it is discussed. It looks for early change that is not yet a tumour. After treatment, every man is shown how to check the remaining testis and asked to do it each month.
A testis cancer should never be removed or biopsied through the scrotum. If that has already happened elsewhere, bring the operation notes, because it changes staging and follow-up.
A cut through the scrotal skin opens new paths for spread. Instead of the nodes at the back of the abdomen, the groin and pelvis may now be at risk. This cannot be undone, but it can be planned for once the team knows.
Sometimes the same kind of tumour is found in the chest or at the back of the abdomen, with no tumour in the testis. This goes to the tumour board before any treatment, because the plan and outlook differ. Lymphoma of the testis is a different disease: it is treated as a disease of the whole body, not with node surgery. Tumours from other cell types, such as Leydig or Sertoli cell tumours, and tumours in children follow their own separate plans.
For stage I seminoma, close watching (surveillance) is usually preferred. It works only if every visit is kept, so where that is unrealistic, one dose of carboplatin is discussed.
Some men live far from Gujarat, move for work, or have no phone. For them, the single dose is discussed honestly, rather than a check-up plan that is likely to fail. The schedule itself is in testicular cancer follow-up.
Bleomycin can make the lungs sensitive to high oxygen. A man who had it and now needs node surgery has a lung test first. The anaesthetist is told in writing. Oxygen is kept as low as is safe, and fluids are limited during and after the operation.
Sperm banking is offered to every man before treatment, and his answer is written down. These situations do not change that:
The cost is raised at the first visit, so the family has time to arrange it. More is on the testicular cancer surgery and fertility page.
Yes. Bleeding inside a tumour can cause pain. A sudden painful testis is first checked for a twist, within the hour, and a lump is still fully tested even if it hurts.
Not always. In a single testis, removing only the tumour may be possible, with frozen section during the operation and a plan agreed by the tumour board. Sperm banking is offered first.
When hCG is very high and you are breathless, the lungs are in danger. Chemotherapy starts urgently, and the operation follows once you are safe.
Bring the operation notes and reports. A scrotal approach changes which nodes may be at risk, so staging and follow-up are planned around it.
Surveillance only works if every visit is kept. If that is not realistic, one dose of carboplatin may be discussed for stage I seminoma, as an honest alternative.
Testicular cancer as a whole is covered on the testicular cancer surgery page.
